I have Medicaid and HCBS Long Term Care, how do I start services with your agency?

If you have been approved for Medicaid and HCBS Long Term Care services, you can request Mercy Home Services, Inc. as your agency by contacting and informing your case manager at your county's Single Entry Point agency. 

I have Medicaid, how do I start services with your agency?

If you have been approved for Medicaid but have not applied nor been approved for HCBS Long Term Care services, you will need to apply for HCBS Long Term Care by contacting your county's Single Entry Point agency.

I do not have Medicaid, how do I start services with your agency?

The initial step would be to apply for Medicaid by visiting your local county human services office or by applying at PEAK.  Afterwards, you will also need to apply for HCBS Long Term Care services. (NOTE: Medicaid is also known as Health First Colorado)  

Do I qualify for Medicaid?

To qualify for Medicaid, you must meet monthly income guidelines. For more information, please visit the State of Colorado website. You can apply by contacting your local human services office or via PEAK.

Do I qualify for HCBS Long Term Care services?

To qualify for HCBS Long Term Care services, you must already be approved for Medicaid.  For other qualifications, please contact your local county human services office or Single Entry Point agency.

What is my county's Single Entry Point agency?

Adams, Arapahoe, Denver, Douglas, & Elbert County : Colorado Access

Boulder, Broomfield, Gilpin, & Clear Creek County : Adult Care Management, Inc. 

Jefferson County : Jefferson County Options
